Includes:
- (Upstream) Fix dangerous compile warnings
- Fix max image height to prevent spurious page breaks
- Hide <epub:case> in <epub:switch>, show only <epub:default>

Adds a style tweak to show <epub:case> and hide <epub:default>
for the curious person.
